RE:    Summary of violations committed by the following project assistants:

a. Jeffrey Seaton
b. Celine Brownie

Dear Mr. Sylvian:
This is to inform you that two of our project assistants with the above names have deliberately violated the rules and policies agreed upon in the organization. The nature of their offense is hereby stated.
In a little background, Mr. Jeffrey Seaton, the project assistant stationed in the northern portion of the city, did not report in his post for three consecutive days dated September 16-18, 2013 without asking permission from me verbally or leaving any written note in my office to explain his absence.

Ms. Celine Brownie, on the other hand, was not able to submit the necessary official receipts and payments of receivables to the company cashier that were made by her customers amounting to $200 last September 14, 2013.

Mr. Sylvian, these are absolutely cases of negligence of duty as stated in our company code of conduct. As their direct project supervisor, I am appealing to your office for urgent action on these serious matters.

They really failed to inform me of what has been going on despite the fact that they have known our office number and even my personal contact number. Since they are under my responsibility, I’m now officially submitting this report to you for appropriate sanctions. I shall respect whatever decision you have placed upon them.

Subject: VIOLATION OF TWO PROJECT ASSISTANTS

This is in response to your request dated September 19, 2013. Please instruct the two affected individuals, namely Jeffrey Seaton and Celine Brownie, to report immediately here at the main office.

They are required to submit letters or any form of note explaining their respective situations. The office exercises unstained objectivity and impartiality regarding these cases. As such, we will give them the chance to explain their sides and after that, they will be given the appropriate sanction. If they have valid reasons, the organization still welcomes them with open arms and pretends that no one has done wrong. But if they failed to convince the members of the grievance committee, then our decision may lead to the termination of their contract of employment and cancellation of their names in the payroll list, and they shall get no employee benefits. Worse, this may subject them to court proceedings, a scenario which we hate to happen supposedly.
We do not tolerate this kind of incidents. Thus, the main office promises to exercise fair judgment so that they will become the models to other employees. If they want to stay in the organization, they must abide by its rules and regulations.
